How do I schedule an appointment at Arizona's Vision?

We strive to offer appointments within 1–2 weeks for new patients, and same-week appointments when possible.
What should I bring to my first appointment?
For your first visit, please bring: your current glasses or contacts (if applicable), any existing prescriptions, your vision/health insurance card(s), a photo ID, a list of current medications, and your medical history including any known eye conditions. New patient forms can be completed online to save time on the day of your appointment.
Do you offer emergency eye care appointments?
Yes! If you are experiencing an eye emergency such as sudden vision loss, severe eye pain, a foreign body in your eye, or a sudden increase in floaters or flashes, please call our office immediately at (480) 706-3937. We will do our best to accommodate emergency appointments on the same day.
Is parking available at your Phoenix office?
